Plot

The home ketamine treatment has faded and she is back using her wand and does not want to talk about it, despite the concerns of everyone. House must diagnose Dr. Ezra Powell (played by Joel Gray), a 71-year-old pioneer in the field of medical research who collapsed while studying cancer in mice. House puts Powell through the power of diagnostics, but the team can not produce a conclusive diagnosis, and Powell's health continues to deteriorate. Getting weaker, Powell finally demanded that the team stop the litany of medical tests and put him to sleep. House strikes a deal with him and asks for a day to test him.

The moral dilemma of whether to help suicide and obey Powell's wishes, or ignore what their patients want encourages the team in different directions. Team members have different opinions about morality helping Powell die. Cameron hates the idea of ​​ignoring Powell's wishes and continuing the House's command test, but can not force himself to help Powell in suicide. His inability to make decisions is central to the plot and his character: throughout the episodes team members criticized him for his hesitation. Both Foreman and Cameron oppose watching House put Powell to sleep, but Chase seems to have a bit of a problem. Wilson said that House had done it before "many times" and House insisted that it was only "for patients that [he] knew were terminals."

When House fails to diagnose Powell in one day, she visits him in her room to give her a lethal dose of morphine. Foreman tried to stop him, but House insisted and threatened him with "What will happen here, is that someone will get the morphine buttload.I am not sure who at this point." Foreman and Cameron leave in shock and disgust, but Chase still helps. But instead of killing him, House put Powell in a coma, so he could make the diagnosis calmly. However, during a lung biopsy, Powell's lungs came out and House was forced to intervene with which he realized that Powell lost his reflex ability in his right arm. To proceed with additional diagnostic procedures, House built Powell from his coma and discovered that Powell also lost sensation in his abdomen and right leg.

The conflict between Cameron's work and his indecision reached a boiling point when House finally asked him to get a skin biopsy. His rejection, saying they did not know if it was necessary, sparked an explosion from him: "Exactly! You can not decide whether we help or hurt him, whether he is good or bad, or if you want paper, plastic, or gunny sack.. "Cameron agrees, and has a conversation with Powell, where he reveals that he feels things like informed consent interfere with his research and thus he does not let it get in the way. Cameron cut off a piece of skin using his perspective as a solution, in which Powell praised him for doing what he thought was right.

House finally diagnosed Powell with amyloidosis using Congo red, inspired by the red thong view worn by Ali Johnson (played by Leighton Meester), the teenage daughter of a clinic patient, who leads the House to refer to him as his "muse". Chase also discovered that it was sub-type AA and therefore a deadly disease. During the final scene, Powell is seen struggling to breathe on the hospital bed, and Cameron sits nervously in the locker room. The next morning, Cuddy came to the House office to inform him that Powell died at 2:30 am, though it had been declared stable at 2 am. House denies any knowledge and asks Cuddy if he wants to know even if he does. House then went to Cameron, who was crying in the hospital chapel. House tells Cameron that he's proud of him, and leaves.
